This letter serves as a formal legal notice regarding the lack of response to my prior requests for reasonable accommodations under the Americans with Disabilities Act (ADA). Your current inaction exposes the organization to potential legal liability.

Dear Ms. Martinez and Mr. Pierre,

I am writing to provide an update regarding my ongoing medical care and upcoming treatment. I have an appointment scheduled with my therapist tomorrow, October 22, 2024, at 10:00 AM EST. Following this session, I will submit the therapist’s notes, which will integrate with records from my Primary Care Provider (PCP) and Cardiologist to form a comprehensive report detailing my treatment, accommodations, and return-to-work implications. This documentation will be provided promptly and in good faith.

Despite my consistent efforts, MetLife has not responded to my prior inquiries regarding time and pay, ADA accommodations, and case status. Your lack of communication has forced me to seek guidance from federal agencies and advocates to ensure compliance.

The erratic and non-existent management of my time and pay has been extremely difficult to shoulder given my current health crisis. Nevertheless, I remain committed to operating in good faith.

Please find securely attached medical notes from my appointment on September 17, 2024. I look forward to submitting my therapist’s notes promptly following tomorrow’s session. If you have any questions or require further information, please do not hesitate to contact me.
