I am writing to formally request that the following items and actions be included in the records for my EEOC charge, as they pertain directly to the ongoing investigation into allegations of discrimination, retaliation, and failure to accommodate under the ADA:

  1. Workday Audit Logs: Requesting access to all audit trails from Workday for my employee profile, including changes made to payroll, benefits, or accommodation records.
  2. Escalation Records: A record of any escalations or communications related to my requests for ADA accommodations or other employment matters.
  3. Payroll Records: A detailed report of my pay from June 2024 to present, including any discrepancies or adjustments made without notification.
  4. Accommodation Compliance: Copies of all documents submitted by Cox to substantiate compliance with ADA regulations and my physician's specific recommendations.
  5. Performance Documentation: All performance records, including reviews, disciplinary actions, and metrics, that may have been used to evaluate my role during this period.
  6. Corporate Security Inquiry: An investigation into potentially unauthorized changes to my Workday profile, which have been reported to Cox corporate security.
  7. Training Documentation: Details of any training sessions or materials provided for ADA compliance to managers and HR representatives handling my case.
  8. Correspondence Logs: All emails and formal communications between Cox and MetLife related to my disability claim and accommodations.
  9. Policy Adherence Reports: Evidence of adherence to Cox's Short-Term Disability and Return-to-Work policies, including any deviations from protocol.
  10. ADA Interactive Process Logs: A record of all steps taken by Cox in the interactive process to address my accommodation needs, as required by law.

In addition, I am notifying Cox Communications via this letter that I will be requesting the above-listed records directly from them, and I expect their full cooperation. The inclusion of these records is critical to ensuring the accuracy and completeness of the investigation.

Please be advised that any future communications regarding this matter should be copied to me and my designated representative, Jessica Wagner. Jessica has full authority to act on my behalf during this time.
